Effective feedback from committee: Consider changing “What School do you attend” to “School Name” or “Name of School”.
Consider changing “What grade are you in” to “Grade”.
Consider adding a text book that says “Please explain” or “Please provide a brief explanation” under your scale questions. That way students can explain why they answered with that number on the scale.
The survey is fairly long and students attention spans for voluntary (ungraded) surveys is low. Consider using 2 or more surveys with a maximum of 10 questions. Give separate surveys to various students, that way you get all of your questions answered without having to overwhelm the people taking the survey.
